Payment

Registrations will not be processed until payment is received. Checks should be made payable to: Garrison and Associates, Inc. and postmarked no later than August 5, 2009. 

We will accept VISA and MasterCard. Charges will be processed under the name of Garrison and Associates, Inc.

Purchase Order
Individuals/agencies utilizing a purchase order must include an original copy of the order made payable to Garrison and Associates, Inc. with a signature, PO Number, and billing address in order to be processed.  The issuance of a purchase order to secure a registration slot is an obligation for payment whether the participant attends, cancels, or is absent from the conference.

Purchase orders should be mailed with registration form to Garrison and Associates, Inc. or faxed to 614.273.1401.

Registration fee includes: admission to pre-registered sessions, educational materials, continuing education credit, conference food functions, and exhibits.
